MEDICAL ALERT CARDS
We at the Lewy Body Dementia Resource Center have created an LBD Medical Alert Card. This should be taken with you to any emergency hospital visits and a copy left visibly at home for home doctor visits. It can be downloaded from our home page (in the middle of the page on the left side) at: https://lewybodyresourcecenter.org/

DNR’s – “Do Not Resuscitate” Order
It is suggested that this important decision be made ahead of time (along with a health care proxy) and signed. A DNR states that, in the event of a cardiac or respiratory arrest, no attempt is to be made to perform cardiopulmonary resuscitation. Under certain circumstances, another person, as a surrogate or Health Care Proxy, may consent to a DNR for an individual who lacks decision making ability. See LEGAL & FINANCIAL PLANNING under our CAREGIVING & SUPPORT heading for additional information.
