We at the Lewy Body Dementia Resource Center have created an LBD Medical Alert Card. This should be taken with you to any emergency hospital visits and a copy left visibly at home for home doctor visits. The Emergency and Evacuation Assistance Program (EEAP) is for eligible individuals in Miami-Dade County who need evacuation assistance and/or sheltering or who would like to receive a wellness call after a disaster. Please register by calling the above number or by downloading this application: http://www.northmiamifl.gov/docs/EEAP_application_english.pdf . The program offers:
- specialized transportation
- safe shelter
- medical monitoring
- wellness checks

Alzheimer’s Community Care
ACC has a 24-hour Alzheimer’s crisis line at 800-394-1771, which puts caregivers in touch with professionals who can help defuse emergency situations. This also includes the Lost on Foot Silver Alert Program. “The Lost on Foot program is so important to help caregivers understand the risk of their loved one with Alzheimer’s disease wandering away from home.”
It also provides training to staff who work in specialized adult day centers and training to law enforcement personnel and the community at-large.

DNR’s – “Do Not Resuscitate” Order
It is suggested that this important decision be made ahead of time (along with a health care proxy) and signed. A DNR states that, in the event of a cardiac or respiratory arrest, no attempt is to be made to perform cardiopulmonary resuscitation. Under certain circumstances, another person, as a surrogate or Health Care Proxy, may consent to a DNR for an individual who lacks decision making ability.
